    bitspire ADR-005 rollout step 2, slice 1 (#49). A cash_out settlement
    lands as awaiting_dispense and waits for the machine's report_dispense
    instead of being distributed the instant the payment lands: confirmed →
    distributes; some notes out → partial_pending, held whole; nothing out →
    cash_owed, first on the worklist. Append-only dispense_reports, the
    hold mirrored onto the machine row, a Resume cash-out button, and the
    partial-dispense dialog pre-filled from the hardware's own count.

    Migration m016 (additive). Pairs with aiolabs/bitspire#123.
